3rd Party Information Access Request Form

This form will allow you to send an email to your student requesting access to their information.
Both email addresses must be complete for the message to be sent.
Here is a list of all the available options. Please check all that you wish to request access from your student.

The following text will be sent to your student. You may also enter a personal note below.

"A person with email address (your email address) has requested that you grant him/her access to your student information in order to view academic information or make a payment on your student account. To grant a third party access to your student information, you will need to visit myOC at my.oc.edu/default/proxy"
